Please watch the 2 1/2 minute video and fill out the Clinic Evaluation Below before your next visit. We haven’t updated the old video from when we used to do this in the office on paper. With that being said, we still haven’t figured out how to keep these anonymous for those who wish when filling them out electronically. So, if you want to remain anonymous just ask for a sheet when you come in for your next appointment and you can drop it in the mail to (1330 Ridge Rd - without a return address).
