Instructions:

  1. Fill out your patient information accurately. (Be sure to include any Promo Code)

  2. Click Continue.

  3. Choose a date & time that fits your schedule.

  4. Our office will contact you to verify your appointment and answer any questions you may have (directions, insurance etc). Please respond to our verification call, text or email. All appointments not verified will be removed from our schedule to open up the appointment slot for other patients in need.
